PADI Diver Medical Form & Clearance Guide
At International Diving Centre, your safety during every dive is our top priority. Scuba diving is an exciting, physically rewarding activity that requires good overall physical and respiratory health. To ensure you are fit to dive, all course participants must review and complete the official PADI Diver Medical Participant Questionnaire.
IMPORTANT: Mandatory Medical Clearance Before Pool Training
Your completed PADI Medical Form MUST be fully submitted and approved BEFORE you enter the pool or ocean for in-water training! If physician clearance is required based on your answers, you cannot participate in any pool session until a physician has signed and dated Page 3 of the form.

The PADI Diver Medical Questionnaire follows strict medical screening protocols to determine if you can self-clear or if a physician's sign-off is mandatory:
- Questions 3 and 10 (Automatic Physician Clearance): If you answer YES to Question 3 (lung/breathing issues) or Question 10 (heart/circulatory issues) on Page 1, you MUST have a licensed physician evaluate you and sign Page 3 before diving.
- Other Questions (Go to Page 2): If you answer YES to any other question on Page 1 (Questions 1, 2, 4, 5, 6, 7, 8, or 9), you must go to the designated detail box (Boxes A–G) on Page 2.
- Page 2 Follow-Up Answers: If you answer YES to ANY follow-up question on Page 2, physician evaluation and sign-off on Page 3 is FOR SURE REQUIRED prior to in-water training. If all follow-up questions in the indicated box on Page 2 are answered NO, you may self-clear.

Steps for Physician Approval (If Required):
- Bring All 3 Pages: Take the entire document to your physician—including Page 3 (Diving Medical Guidance), which outlines key conditions relevant to hyperbaric environments.
- Physician Sign-Off: Your physician must check the box declaring you APPROVED to dive, sign, and date Page 3 (Medical Examiner's Evaluation Form).
- Submit Prior to Pool Sessions: Email or hand in your completed original signed document to International Diving Centre prior to your first scheduled pool class.
